EXCIMER LASER TREATMENT FOR RECURRENT INJURIES (type PTK) 

PROCEDURE

- 1 WEEK BEFORE : take off your soft lenses (note: a hard contactlens must be removed 2 weeks before surgery)

THE DAY BEFORE
 Get all the necessary medication: Valium 5 mg (sedative), / Trafloxal eyedrops (antibiotics), FML eyedrops  (inflammatory), Aculare eyedrops (inhibits pain) and Dafalgan codeïne and Valtran oral solution (against the pain). You also have to arange a driver that can bring you tomorrow.
  

- THE DAY OF THE OPERATION

2H before your appointment: no more eating (you can drink) /remove mascara and make-up.
 ½ H  before your appointment: let 1 tablet temesta melt under your tong  - you receive the tablet when you arrive in the lasercenter.
* what to take with you to the lasercenter: unopened bottles:  Aculare en Trafloxal bottle. The procedure takes about half an hour// possibly some waiting, because there are also other patient who need to be treated.
 
- DURING THE OPERATION

A special deviece will hold your eyelids open, so you can't blink unexpected.
You will have to look to a red light.
You will first see a green light.
It is very important that you keep looking into the red light, when the doctor asks you to do that.
First the doctor will scrub away the superficial layer of the cornea (this takes a couple of minutes).
Then the laser is activated: you will hear a crackling noise, usually accompanied by a strange burning smell.
This is due to the evaporation of a portion of your cornea, this is normal.
Immediately thereafter they put on a contactlens, to fight the pain. 

  - AFTER SURGERY

You will have to come every day to the ophthalmologist till the injury is healed.
Afterwards you will have to visit you ophtalmologist a couple of times to evaluated the healing process (and adjust if necessary).
6 months after surgery the last control is planned.
Meanwhile you will have to use some medication and be carefull for your eyes.
In the treatment you can distinguish 2 periods: the healingphase of the superficial injury (1°week) and the healingphase of the applied deformation (6 months): 

             A. INJURY PHASE OF THE SUPERFICIAL INJURY

* Medication :
- Trafloxal eyedrops every 2H ( not at night ): disinfection
- Aculare eyedrops: 4 X/day  ( cyclopentol only once )
- Possibly your ophtalmologist will apply a bandage instead of a contactlens. Afterwards you don't have to apply the medication yourself.
  * Variabele medication :
Your eye will hurt a little bit. Variabel painkilling is therefore very important:
a) Position : Don't look around too much and close both eyes. Try to sleep as much as possible.
If you can't sleep because of the pain, you may take a tablet of valium. Listen to some music, while your awake.
b) Oral medication:  Dafalgan when the pain is moderate / when the pain is intense: Valtran drops (=  10 a 15 drops 3 a 4X/day).
Keep eating on regular base, so the painkillers won't affect your stomach.
c) Medication in eyedrops : additional Aculare instillation : 2 a 4X /day
  * Other guidelines during the first days:
- It can't hurt that you have running eyes.  Tears are a disinfection. Do not rub your eyes, otherwise you rub the lens out and you can cause infection! 
- It's normal  Het is normaal dat u uw oog nog niet goed kunt openhouden. Best draag je een zonnebril
- uw zicht zal slechts beter worden als de kwetsuur genezen is ( duurt ongeveer 1 week)
 
            B.  HEALINGSPHASE
- When the injury is cured you need to use the ointment in the evening . When a haze arises in the cornea (this mostly doesn't happen), you need to use additional FML drops see this schedule : 
-  4X/d during 1° week
- 3X/d diring 2° week
- 2X/d during 3° week
- 1X/d during 4°  tot 8 °week
* when you experience a light irritation, you may use intermittent  Vidisic eyegel (lubricating substance)
* depending on the evolution of your vision without correction the schedule may be changed, or there can be decided to use lighter of stronger drops.    
- In the beginning your will suffer to read because you have a little over correction. This will disappear in the coming three months.
- If your glass is too storng on the operated sight, it's best tolet it remove by your optician. In case you have a strong correction (more than -3) the difference between both eyes will be big and u couldn't wear your glasses comfortable. The eye that isn't operated yet, can be corrected with a disposable lens. (one that can be worn day and night) . You have to remove this lens a week before surgery.
Watch out with driving your car, because your depth vision is'nt ideal. As long as your both eyes aren't corrigated...
- wait for one month to swim / 1 week no make-up / wearing sunglasses by bright sunlight
- after 6 months: the result is definitive
